Brighton & Hove Service Delivery Launch Monday 5 December 2022
On Monday 5th December over 60 people from a wide variety of organisations gathered on the fourth floor of Bart’s House to launch the Multi-Disciplinary Team for Changing Futures in Brighton and Hove.

Representatives from Brighton and Hove City Council (BHCC) were joined by representatives from the voluntary and community sector, as well as the formal partnership organisations involved in the Changing Futures Multi-Disciplinary Team.
The Operations Manager, Holly Croydon, welcomed guests to the Launch event. She added,
“The operations team are steadily growing, and we are excited to begin working with clients in this wide-ranging programme addressing those in our community with the most complex needs. We look forward to working with you!”
The Changing Futures Multi-Disciplinary Team is made up of workers from Adult Social Care, BHCC Housing, Change Grow Live, Just Life, Rise Domestic Abuse and Sussex Partnership Foundation Trust. They are tasked with working to improve the lives of those with the most complex needs in Brighton and Hove.

Working with a philosophy of collective responsibility, the team will hold smaller caseloads so that intensive work and support can be offered in areas of housing, substance misuse, health and social care alongside a string Peer Support element. This ‘no wrong door’ approach will, it is hoped, create more joined up services and prevent the recurring problem of clients being retraumatised by having to repeat their story to individual organisations.

The Brighton and Hove Multi-Disciplinary Team is part of the Pan Sussex Changing Futures initiative which works not only operationally, but at a level of systems change, aiming to create change at the individual, service and system level.
